Hi Dr. Potter I just wanted to get your opinon in regards to having an embryo transfer on day 6 (instead of 5) where the PGD is performed on day5? If given the option is this something to recommend? What are the pros and cons to this? I have read there is a higher lots of success rates with this plan!!! ...just not sure what to do?
You are talking about blastocyst biopsy. There are still a number of problems with blasocyst biopsy in my opinion when using FISH.
Pros: Many cells to evaluated instead of just one. Potential for better accuracy and avoid false or insignificant mosaicism
Embryos biopsied only after proving metabolic competence. Fewer embryos biopsied. Less work in the lab. Decreased cost?
Cons: High rate of multinucleated cells make FISH even more unreliable that usual. High percentage of abnormals.

When GeneSecurity is introduced it may help reduce the concerns related to multinucleated cells. There are certainly no data to support a hihger pregnancy rate.
