Best way to go? Aggressive or low stims?

My first cycle we did a normal stim (or normal compared to the second one) and the second one, we went more aggressive and bumped up drug dosages and added another into the mix. End result was half as many eggs retrieved than expected, less the second cycle than the first, but oddly enough, the same amount of embryos sent for PGD.

I'm now reading about studies done where they compared high and low stims and even though low stims resulted in fewer eggs retrieved, they were of a better quality. Does anyone have information to support this from what you've gone through? As much as I want more eggs to fertilize, all these drugs are helping to mess up my eggs' chromosomes. I want to give them the best chance possible to be "normal".

All I can say is from my own experience.  My first cycle, the doc was very cautious and I had low stims, only produced about 10 eggs, 9 were mature and fertilised and of those 5 were normal.  My next cycle my doc went a little heavier on the drugs but I way over stimmed and ended up with about 40 follicles, we still retrieved the same amt of eggs but they were not as good quality, I only ended up with 4 normal even though there were all those follies on the u/s.

Thankfully for me, I did get my son from that cycle, but it was just sheer luck.  I do believe less is more.

 If you look through all of the PGD stats on here, you will see that a lot of the time ladies who retrieve 20+ eggs still only end up with a similar amt of normals at the end compared with those who only retrieve around 10 eggs.  In saying all of that, you don't want too few eggs either 'cause you need a few to work with, I hoped for around 12, not too many, but enough.

 I agree that having a little less is better, but who knows? In my first cycle I had 27 eggs at ER, 19 mature, 14 fertilized, 7 normal embryos-2 girls and 5 boys(PGD-5 probe). The girls turned out to not be healthy at all and I lost both at different cycles. At this second and last fresh IVF I ended up with even more eggs-31, 27 were mature, 19 embryos on Day 3, 13 became blasts on Day 5 and 6 were GSN normal from a Day-5 biopsy. Unlucky for me, I only have one heathy girl from the bunch, the rest are boys, but 6 GSN normal embryos from that many eggs at age 36 is pretty awesome. I will be transfering her sometimes before mid-July and just cross everything for it to work since it's my last chance. Good luck and I hope it works out for you!

sometimes a tweak in meds is the magic bullet, but I'd have to agree with PP, quantity does not always equal quality. How to get that is the Million $$$ question. GL!!!

dream, that's a lot of eggs! But I understand about the quality.

I am most likely going with low stims this next cycle, it just makes more sense. Of course, I'll have to work with the dr. on that, but going more aggressive turned out to be a major fail.

My dr prefers to use the "low and slow" stime approach and said he wanted quality over quantity and it did work in my case.he said the risk with stimmimg too high and too fast is poor quality,sometimes not quite mature eggs..it did work in my case so I would always say go for the low and slow approach..

How many eggs did you get the first cycle, and how many embryos did you end up with going to PGD both times?

I think in your case you definitely want to do less stims than the second time.  Sounds like the bump up in meds did absolutely nothing for you.  Is there a middle ground between the low dose in cycle one and the high dose in cycle two, or do you think you'd go even lower than cycle one?  What does your doctor say?  When is your next cycle?  GL, and lots of blue dust!

First cycle we did average stims. We got 12 eggs, 11 fertilized, 10 biopsied. Of those: 7 made it. 2 XY, 2 XX and 3 combo of XY and XX but all had chromosomal abnormalities (5 probe done).

Second cycle aggressive stims yielded 11 eggs, 9 fertilized, 7 sent for biopsy. 5 XY, 1 XX (low grade) and 1 empty. Of the 5 boys (which still stuns me) 3 were monochromatic 18, 2 hb appeared normal (5 panel probe again) but both blighted - 1 confirmed trisomy 10.

I've spent since March getting healthy. You'd be amazed at my journey! Downside to everything is, I have Hashimoto's Thyroiditis and am being screened for Celiac. End result after all healthy eating is no more gluten. I was going to try again in October, but decided December might be better. Hubby doesn't want to try again if this cycle doesn't work. If I can't get this to work, nothing else I try would work I fear. Still researching everything!!!
